Brookhaven Hospital is one of Silent Hill’s most iconic locations and while it has retained much from the 2001 original game, the version seen in Silent Hill 2 Remake does have some changes and additions. How to Solve the Brookhaven Hospital X-Ray Puzzle in Silent Hill 2 Remake
You can find the X-Ray Puzzle in the appropriately named X-Ray Room on the 3rd Floor of Brookhaven Hospital. After getting the Basement Key from the Reception Office and restarting the generator, you can use the elevator to reach the 2nd Floor. Go down the hallway at the center of the 2nd Floor and then go west from the Director’s Office. When you reach the west end of this hallway, go north and then turn right at the end of this hall to find a staircase that leads to the 3rd Floor.
Turn left when at the top of the stairs and then take another left. At the west end of this hall, go south. Exam Room 4 can be found on the left side of this hallway. Break the window and climb into the Exam Room. Go through the door to enter the X-Ray Room. On the wall is the X-Ray Viewer where you find the puzzle. Two Radiographs are on the board with a third one, a Radiograph of the Pelvis, on a nearby operating table along with a note that says the following: “Dr. Andrews, we delivered the radiographs you requested. However, it looks like the file was incomplete, as one of the pictures was nowhere to be found. We will send somebody as soon as we can to locate it. Let me know if you need anything else.”
Before leaving the area to track down the last Radiograph, make sure to go to Exam Room 5 and grab the 1F Inner Ward Key since it will be needed to access the part of the 1st Floor with an important item to make the Radiograph usable.

Pass through the Station and go into the Treatment Room to find a bathtub crawling with bugs. Inside the tub is the Moldy Radiograph. This won’t be usable until it is cleaned. This means you need to find Mold Remover.
Go back to the 1st Floor and head to the lobby. Go up the hallway at the center of the lobby and unlock the door at the end of the hall with the 1F Inner Ward Key. Turn left at the door and then go right at the end of the hall. Take the next left and go into the Pantry. In the corner right at the border between the Pantry and Kitchen is a trash chute. The Mold Remover is found here. Go into your Inventory and Combine the Moldy Radiograph and Mold Remover to get the Cleaned Up Radiograph showing a hand.
With all the Radiographs, you can now complete the X-Ray Puzzle. Your task is to line up the Radiographs with the markings on the viewer which will then reveal a code. The code and placement of the Radiographs are the same on all Difficulties with the only difference being that the Hard Difficulty has fewer markings to help you in lining up the images.
- The X-Ray of the skull needs to be vertical to line up the line that has “Skull Trauma from childhood” written next to it.
- Rotate the pelvis to the right so that it looks like it’s standing up straight and have the left side of the image overlay the skull to reveal a number. On Light and Standard Difficulties, this is made easier with the 3 lines and X mark that can be lined up.
- The Cleaned Up Radiograph should be turned to the left, and then the top left of the image should be overlayed with the pelvis image to reveal a number once again. On Light and Standard, this can be made easier by lining up the two black lines at the bottom of the image.
- Turn the image of the chest to the left and line up the two halves of an X on the right side of the image to reveal the final number.
The Code will be the same on every Difficulty: 4-37-12
This code is used to open the padlock on Room D1 right across the hall from Exam Room 4. Use this code and turn the dial left, right, and left to open the door and find the Filthy Bracelet for the Director’s Hand Puzzle.
