These are so rare, the Diablo 4 developers had to detail them for the community. These 6 items are rare even compared to Uniques and were once thought impossible to farm in any targeted way. But there is a way to boost your finds. You might be able to get an Ultra Rare Unique following the techniques listed below.
Alternatively, you can farm Ultra Rare Uniques by doing what everyone else does — pray for the best. Right now, the best Unique farming methods are by completing Endgame activities. Unlocking Mystery Chests in Helltides can give you a pretty good random shot at finding a Unique, just like completing high tier Nightmare Dungeons. But, by defeating specific types of enemies, you can target which Ultra Rare you want. Instead of getting an extremely low chance of getting any random Ultra Rare Unique, try target-hunting just one with the right enemy group listed below.

How To Target Farm Ultra Rares
Uniques are the best loot in Diablo 4 — and they’re also incredibly rare. While hunting Mystery Chests in Helltides is still one of the best ways to farm for Uniques in general, there’s a more targeted method for hunting the uniques you actually want. Certain enemy types will drop certain Uniques. Bandits drop certain uniques. Cultists drop other uniques. Cannibals drop yet different uniques. Specific types of uniques will only drop certain gear, and by farming Nightmare Dungeons with the right kinds of enemies, you’ll be able to target the type of gear you want.
- How To Farm Specific Uniques: Certain enemies have increased chances of dropping Uniques. By farming Nightmare Dungeons at the highest Tier possible — and targeting Elite enemies — we can target specific Uniques.

NOTE: Ultra Rare Uniques are only available on World Tier 4 at Level 85+. Ultra Rare Uniques won’t drop any other way. Ultra Rare Uniques are also much rarer than standard Uniques.
Ultra Rare Uniques | Best Dungeons To Farm
Farm these types of enemies in Nightmare Dungeons for a better chance at finding Ultra Rare Uniques. The higher the Nightmare Dungeon Tier, the better your chances will be.
- Andariel’s Visage [Unique Helm]: Lucky Hit: Up to 19%[15-20]% chance to trigger a poison nova that applies 10,077 Poisoning damage over 5 seconds to enemies in the area.
- Dropped By: Cultists and Cannibals
- Cultist Dungeons: Conclave, Cultist’s Refuge, Serpent’s Lair
- Cannibal Dungeons: Earthen Wound, Guulrahn Slums
- Doombringer [Unique Sword (Stabbing)]: Lucky Hit: Up to a 20% [15 – 25]% chance to deal 5,822 damage to surrounding enemies and reduce their damage done by 20% for 5 seconds.
- Dropped By: Knights, Skeletons and Zombies
- Knight Dungeons: Crusader’s Cathedral, Renegade’s Retreat
- Skeleton Dungeons: Belfry Zakara, Crusader’s Cathedral, Demon’s Wake, Kor Dragan Barracks, Raethwind Wilds, Sunken Ruins
- Zombie Dungeons: Earthen Wound, Guulrahn Canals, Guulrahn Slums, Kor Dragan Barracks, Onyx Hold, Shivta Ruins, Sunken Ruins
- Harlequin Crest [Unique Helm]: Gain 13%[10-20]% Damage Reduction. In addition, gain +4 Ranks to all skills.

- Cannibal Dungeons: Earthen Wound, Guulrahn Slums
- Melted Heart of Selig [Unique Helm]: Gain +30% Maximum Resource. In addition, when you take damage, drain 6[8-3] Resources for every 1% of Life you would have lost instead.
- Dropped By: N/A – Extremely rare. Farm Mystery Chests in Helltides for the best shot.
- Ring of Starless Skies [Unique Ring]: Each consecutive Core Skill cast reduces the Resource cost of your next Core Skill by 8% [8-12]%, up to a maximum of 40%.
- Dropped By: Ghosts
- Ghost Dungeons: Abandoned Mineworks, Ancien Lament, Earthen Wound, Guulrahn Slums, Lost Archives, Onyx Hold, Sunken Ruins
- The Grandfather [Unique Two-Handed Sword (Slashing)]: Increase your Critical Strike Damage by 76%[x][60-100]% The other properties on this weapon can roll higher than normal.
- Dropped By: Knights and Zombies
- Knight Dungeons: Crusader’s Cathedral, Renegade’s Retreat
