In addition to making the game just harder in general with more lethal combat, the survival difficulty adds encumbrance to ammo, removes enemies from appearing on your compass, and adds a new Adrenaline perk that boosts your damage output the longer you stay awake and accumulate kills. However, sleeping will lower your Adrenaline.
The biggest addition is the “Wellness” feature, which requires you to stay healthy, fed, and rested. Being unwell will reduce your statistics and add to your Fatigue, which affects how many action points you have. Your Fatigue rating works like Radiation sickness but affects Action Points instead of Hit Points and shows up as a red block on your AP bar. Your immunity rating will also be lowered and can eventually deal physical damage to you.
It also disables both manual and quicksaves, making it only possible to save when you’re in bed and sleep for at least an hour. You won’t be able to save scum before boss fights. You won’t be able to fast travel anymore, either–so expect to do a lot of walking and planning out your routes in the Commonwealth.
